I was diagnosed with diabetes when I was 8 years old and I have to admit looking back on my life I wasn’t as dedicated to eating right and exercising as I should have been.

As the doctor explained it to me, diabetes damages the small blood vessels in the body and when the blood vessels in the kidneys are injured it results in your kidneys being unable to properly clean your blood.

As a result, your body starts retaining more water and salt than it should, which results in weight gain and ankle swelling. In addition you may find protein in your urine and waste materials may also build up in your blood.

None of this is good for your health. Plus, diabetes can also damage your nerves. This is called diabetic neuropathy and one of the affects of this condition can be difficulty emptying your bladder. The pressure from a full bladder can then back up and further injure your kidneys. You may also develop an infection from urine that remains in the bladder too long.

Some of the early warning signs of diabetic kidney disease are an increased excretion of albumin in the urine, weight gain, ankle swelling, high blood pressure and the urge to urinate at night.

Later, as your kidneys begin to fail your blood urea nitrogen (BUN) levels will rise as well as the level of creatinine in your blood.

In addition you may also experience nausea, vomiting, a loss of appetite, weakness, increasing fatigue, itching, muscle cramps and anemia.

I Was Experiencing Several of These Symptoms
When I Went to See the Doctor

I still remember sitting in the doctor’s exam room as he told me the news –that I had diabetic kidney disease and that there was a very good chance that I would suffer total kidney failure in the not too distant future.

My heart sank and I felt sick to my stomach. I left the doctor’s office that day in a fog and the first thing I did when I got home was start searching the Internet for more information on diabetic kidney disease.

What I found there honestly scared me to death.

I read stories of intense suffering, of constant “brain fog” and moodiness, fatigue, swelling, headaches, depression and loss of appetite.

I read stories of the pain and discomfort of dialysis treatment. I read stories about having a fistula put on your arm so that you can undergo the dialysis treatments – that was something that I had never heard of before and I wish to this day that I never had heard of it.

I was shocked and worried.

I didn’t want to go through any of this. I didn’t want to feel tired and depressed. I didn’t want my legs to swell and I didn’t want to have transplant surgery.

At that time my future looked so bleak and scary that I broke down and cried.

I cried and I cried until I literally couldn’t cry any more. Then I just sat there, feeling spent, wondering what I was going to do. I asked myself what can I do? I really didn’t know what options I had, so I began doing more research online.

What I discovered during those long Internet searches ended up changing my life and saving me from the pain of long needles and cramping and all the other negatives that accompany dialysis.

Blood Sugar Level Ranges

Understanding blood glucose level ranges can be a key part of diabetes self-management.

This page states ‘normal’ blood sugar ranges and blood sugar ranges for adults and children with type 1 diabetes, type 2 diabetes and blood sugar ranges to determine people with diabetes.

If a person with diabetes has a meter, test strips and is testing, it’s important to know what the blood glucose level means.

Recommended blood glucose levels have a degree of interpretation for every individual and you should discuss this with your healthcare team.

In addition, women may be set target blood sugar levels during pregnancy.

The following ranges are guidelines provided by the National Institute for Clinical Excellence (NICE) but each individual’s target range should be agreed by their doctor or diabetic consultant.

Recommended target blood glucose level ranges

The NICE recommended target blood glucose levels are stated below for adults with type 1 diabetes, type 2 diabetes and children with type 1 diabetes.

In addition, the International Diabetes Federation’s target ranges for people without diabetes is stated.

Blood sugar levels in diagnosing diabetes

Random plasma glucose test

A blood sample for a random plasma glucose test can be taken at any time. This doesn’t require as much planning and is therefore used in the diagnosis of type 1 diabetes when time is of the essence.

Fasting plasma glucose test

A fasting plasma glucose test is taken after at least eight hours of fasting and is therefore usually taken in the morning.

 

The NICE guidelines regard a fasting plasma glucose result of 5.5 mmol/l as putting someone at higher risk of developing type 2 diabetes, particularly when accompanied by other risk factors for type 2 diabetes.

Oral Glucose Tolerance Test (OGTT)

An oral glucose tolerance test involves taking a first taking a fasting sample of blood and then taking a very sweet drink containing 75g of glucose.

After having this drink you need to stay at rest until a further blood sample is taken after 2 hours.

HbA1c test for diabetes diagnosis

An HbA1c test does not directly measure the level of blood glucose, however, the result of the test is influenced by how high or low your blood glucose levels have tended to be over a period of 2 to 3 months.

Indications of diabetes or prediabetes are given under the following conditions:

  • Normal: Below 42 mmol/mol (6.0%)
  • Prediabetes: 42 to 47 mmol/mol (6.0 to 6.4%)
  • Diabetes: 48 mmol/mol (6.5% or over)

Ways to prevent osteoarthritis knee pain in adults

New research finds that an intensive exercise and healthy eating regime can reduce the knee pain caused by osteoarthritis in overweight adults with type 2 diabetes.

Osteoarthritis is a common complication for overweight people with type 2 diabetes. The added weight places extra strain on the joints. Osteoarthritis can also develop as a result of natural wear-and-tear, so it tends to affect older people more often. Being overweight and over the age of 50 are also conditions that increase the likelihood of type 2 diabetes.

The researchers hypothesised that the right diet and the right exercise regime could prevent knee pain caused by osteoarthritis

“Prior to this study, we did not have empirical data to support the claim that diet and exercise actually worked to prevent knee pain,” said study leader Daniel White, Assistant Professor of Physical Therapy at the University of Delaware.

“Now we do have a study.”
How was the study conducted?

The researchers compared two groups: the first received intensive lifestyle intervention (ILI); the second received standard diabetes mellitus support and education (DSE). After one-year and four-year periods, the participants were questioned about any knee pain they experienced.

“The analysis involved a subcohort of 2,889 subjects who reported no knee pain at baseline, but were at high risk due to obesity.”

The intensive lifestyle intervention mainly involved restricting calories. Moreover, at least 10 per cent of the participants’ calories had to come from protein. Exercise intervention was based on gradual progression of home-based exercise, culminating in 175 minutes of vigorous exercise a week. In most cases, the particularly activities included brisk walking.

White explained that the research only focused on people with type 2 diabetes who were overweight, because the excess weight increased the strain on their joints. People with type 2 diabetes triggered by other causes, such as polycystic ovary syndrome and genetics, do not have a heightened risk of osteoarthritis.

“We did not study people in the general population, but only adults who were diabetic and overweight,” said White.
The results

White found the results reassuring. People who received the intense lifestyle intervention were 15 per cent less likely to report knee pain than those who received the standard diabetes mellitus support and education.

“Among these we studied who were randomised to the diet and exercise intervention, it was found that they were 15 per cent less likely to develop knee pain compared with their counterparts randomised to the control condition.

“These findings are very important. They demonstrate that the recommendations to exercise and diet to make a difference for preventing the development of knee pain among those who are at high risk.”

Knee pain is responsible for more disability among people of middle or older age than any other condition. These findings could lead to more effective methods of knee pain prevention.

“I study physical activity among people with knee arthritis,” White said. “I felt it was important to investigate whether exercise combined with diet did in fact protect against the development of knee pain.”

The findings were published in Arthritis Care and Research.
